Instructions for Completing a Swim Meet Entry Card

  1. ID No.
    This number comprises the first three letters of a swimmers surname + the first two letters of the first given name + the birth date. Example for John Citizen born on 12 January 1983 is CITJO120183
  2. DOB
    birth date of swimmer
  3. Age first day of meet
    Actual age of swimmer on the first day of the swim meet. (Note: Some meets are held over 2 or more days).
  4. Surname
    of swimmer
  5. First Name
    first given name of swimmer
  6. Phone
    home phone number of swimmer (in the event the Swim Meet organizers need to obtain further details).
  7. Club
    Name of the club in which the swimmer is registered with W.A Swimming Association.
  8. Registrar's Signature
    The Club’s Registrar must check each entry form and then sign before it is sent.
  9. Event No
    This is the number for each of the events the swimmer nominates to enter
  10. Distance
    This is the distance of the nominated events entered on the card.
  11. Qualifying Time
    Some swim meets require a swimmer to have W.A Swimming Association recognized times that are not slower than the Qualifying times. Swimmers aged 7 and 8 years are required to have a Proficiency Certificate to enter some W.A Swimming Association approved Swim meets. Please check with the Club Registrar in regard to Proficiency Certificates. Several swim meets are held where swimmers must have entry Times slower than the Qualifying times. Please check the details for each Swim meet carefully.
  12. Entry Time
    This is the swimmer’s fastest time recorded at a W.A Swimming Association approved meet or at an approved time trial. Entry Times must have been achieved by the swimmer within the last 12 months.
  13. Achieved AT
    Name and Date of swim meet where Entry Time was achieved.

Note: It is the responsibility of the Swimmer or one of their parents to complete entry cards.
